There are many ways in which an investigation can be started, and in less serious cases it is likely HMRC will notify you of an enquiry. Fines and consequences

Penalties and other consequences will vary depending on the specific circumstances of a case, and it should be kept in mind that HMRC will more heavily punish those it believes are being deliberately misleading or committing VAT fraud.

A situation’s severity is worked out on a scale, with carelessness the least serious (you made a genuine mistake without malicious intent) through to deliberate (you filed a tax document you were aware was inaccurate, for instance) and deliberate and concealed. These last two comprise the most serious of cases, and if a business you work with is labelled as such, it will have likely provided false documents and made an attempt to hide this fact.
